Patients are responsible for payment the day of services are rendered including all insurance co-pays and deductibles. We accept MasterCard, Visa, Discover & many debit cards. Our practice participates with most insurance plans.
As a courtesy to our patients, we call and verify benefits on the insurance information you provide.

This quote is NOT a guarantee that benefits will be paid as stated during the conversation. The information provided by the customer service associate may be inaccurate. For this reason Carmel Foot Specialists will not be responsible for any inconsistencies in quotes versus benefits actually paid.
The patient is strongly encouraged to review the benefit plan booklet provided by the employer & or insurer.
